To provide services to patients in outpatient program to include residential, IOP, SACOT. Participates as a member of the Patient Centered Health Home (PCHH) Care Team.
1.To provide services to patients identified with substance abuse problems to include but not limited to:
a.Provides transportation in agency vehicles and comply with agency rules and regulations.
b.Prepares meals and ensures compliance with universal health standards.
c.Assists coordinator as needed in program activities.
d.Maintains appropriate logs.
e.Works on behalf of patients in assuring service delivery.
f.Accompanies patients to outside appointments. Assures that patients understand theprocess and information given and that patient's views and rights are evident and clear toall interested parties.
2.Interacts appropriately with SA Program patients and provides support andsupervision according to rules and regulations.
3.Assists counselors and therapist in assuring services needed are rendered.
4.Assist in clerical duties as assigned.
5.Assists in translating English and Spanish when needed.
6.Ability to work shift-hours as scheduled to include evenings and weekends.
7.Required training: CPR, NCI, Medication administration and other state required training
8.Other duties as assigned.
EDUCATION AND EXPERIENCE: High school diploma and at least 2 years interactive work experience in a human services or health care organization.
LANGUAGE SKILLS: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to communicate effectively before groups or organizations.
A. Assists the patient in development of the patient’s Behavioral Health self management goals and care plan. Documents the patients’ progress with meeting goals. B. Focuses on the assessment, monitoring and proactive management of the healthcare needs of the patient population. C. Participates in case reviews during PCHH huddles: i. Daily ii. Multidisciplinary
communicate in Spanish and English with patients and family members from various educational, socio-economic and cultural backgrounds.
MATHEMATICAL SKILLS: Ability to apply concepts of basic mathematics.
REASONING ABILITY: Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Verbal and written instructions from the Behavioral Health Service Director.
COMPETENCY/SKILL REQUIREMENT: Competency required in the areas of communication, excellent verbal and written skills.
CERTIFICATES, LICENSES, REGISTRATIONS: CPR Certification, current, valid NC Drivers License.
OTHER SKILLS AND ABILITIES: Knowledge of substance abuse issues and 12-step programs. Ability to assess and visually present data. Ability to deal effectively with stsress and to work under presssure..
