Staff Physician Assistant or Nurse Practitioner provides services to the clients at CommWell Health and in the community through outreach and community programs. Participates as a member of the Patient Centered Health Home (PCHH) Care Team.



1.Provides direct outpatient primary care to patients by performing diagnostic and therapeuticservices to assist physician in diagnosing medical and health and prescribing necessary treatmentfor primary patient care;

  records pertinent data for review by physicians.


2.Takes patient history; conducts physical exams, records data; writes progress notes andnarrative summaries.


3.Prescribes medication utilizing clinic formulary for outpatients as feasible and per physician assistant or nurse practitioner protocols.


4.Acts as the liaison with medical care resources when referral is needed per protocols.


5.Requisitions on and off-site lab and x-ray exams as needed per protocol.


6.Manages chronic disease utilizing established CommWell Health protocols and procedures.


7.Participates in continuing education activities on- and off-site when appropriate and feasible.


8.Participates in staff conferences to devise individual patient care plans, to revise and updateclinic services, continuing quality improvement program and continuing education asappropriate.


9.When there are no patients to be attended to, participates in patient related activities.


10.Participates in other related activities affecting the clinic’s programs.




EDUCATION and/or EXPERIENCE: Graduate of an accredited Physician Assistant or Nurse Practitioner  program. National certification as a PA or NP. PA must be licensed to practice by the NC Board of Medical Examiners.  NP must be licensed to practice by the NC Board of Nursing. Ongoing documented continuing education required.


LANGUAGE SKILLS: Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to communicate in Spanish and English to patients and their family members.


COMPETENCY/SKILL REQUIREMENTS: Competency required in the areas of interpersonal communications, excellent verbal and written skills, and knowledge of credentialing criteria and standards.


CERTIFICATES, LICENSES, REGISTRATIONS: Current Physician Assistant or Nurse Practitioner license to practice in North Carolina.
